Delivery of Tax Form 1095-C
In alignment with federal requirements* outlined in the 2025 Paperwork Reduction Act, ·¬ÇÑÉçÇø now defaults to electronic delivery of IRS Form 1095-C, the Employer-Provided Health Insurance Offer and Coverage tax form.
Download Your Current 1095-C
Current, former and retired employees can follow the instructions below to download an electronic copy of their current Form 1095-C.
Active Employees
If you are actively employed by ·¬ÇÑÉçÇø and would like an electronic copy of your Form 1095-C, log in to and select “Your Current 1095-C Form" under the Quick Links menu.
Former Employees or Retirees
If you are retired or no longer employed by ·¬ÇÑÉçÇø, you can request an electronic copy of your Form 1095-C through the . You must authenticate your identity by logging in with a username and password. You can then download an electronic copy of your 1095-C.
Request a Physical Copy
While the university is no longer required to automatically send employees a physical Form 1095-C by mail, you can update your preferred delivery method through the external portal by following the instructions below.
- Log in to .
- Go to the Quick Links menu.
- Select "Your Current 1095-C Form."
- Select "Manage Electronic Tax Statement Options."
- Scroll down to "Electronic Statement Notification Options."
- Select "No" for the first question, "If you are eligible for a 2025 Affordable Care Act (ACA) Form 1095, would you like to access this statement electronically?"
- Click "Save Notification Option Settings" at the bottom of the page.
If you wish to opt back in for electronic access of Form 1095-C in the future, follow the steps above and select “Yes” for the same question.

*These requirements do not apply to current, former and retired employees living in California, New Jersey, Rhode Island or Washington, DC. Employees in these locations will receive a paper Form 1095-C from ·¬ÇÑÉçÇø by mail unless they have opted in for electronic delivery.
