Electronic Form 1095-C Download Instructions

In November, UTSA employees received an email that provided information about opting in for electronic delivery of the IRS Form 1095-C: Employer Provided Health and Insurance Offer and Coverage. .
If you did not opt in for electronic delivery, your paper form was mailed on Jan. 27, 2025. If you did opt in for electronic delivery, Form 1095-C is available to view and download using the instructions below.
Active employees can access their form using the steps below.
- Log in to .
- Go to the Quick Links menu.
- Select “Your Current 1095-C Form”
- Select “Access Current Year-End Statements.”
- Click on the “Download PDF” link to view, print or save your form securely.
Former employees and retirees can access their form using the steps below.

- Select “Access Current Year-End Statements.”
- Select the “Download PDF” link to view, print or save your form securely.
